When it comes to length training, there are some things which can get in the way of progress.

A big mistake many make is not retracting the foreskin (or excess skin) away from the glans before setting a grip or device on the penis. By not doing this, the skin instead of the penile ligaments and tunica takes the brunt of the tension.
